AXA business health insurance: our 2026 review for employers
AXA Health is one of the largest UK insurers for company health cover, and its Business Health plan is one of the most customisable on the market. As a broker, we quote AXA alongside the other leading insurers every week. This 2026 review explains what AXA's business health cover includes, what it costs and who it suits, in plain English.
What is AXA business health insurance?
AXA business health insurance is company private medical insurance from AXA Health, the health arm of the AXA group. A business buys it to give employees fast access to private diagnosis and treatment for acute conditions that start after they join, so they spend less time waiting and less time off work.
AXA's small business product, the Business Health plan, is designed for companies with up to 249 employees. It can cover a limited company with just one or two directors, as well as a growing team. Larger employers move onto AXA's corporate health insurance, which is priced on the company's own claims experience.
Is AXA the same as Bupa? No. They are separate insurers with different hospital lists, benefits and pricing. We compare both, along with Aviva, Vitality and WPA, before recommending anything.
What's covered by AXA's Business Health plan?
AXA builds its cover in modules, so you choose the level of cover your budget allows. The core module is treatment, with options added on top:
Treatment
In-patient and day-patient treatment, out-patient surgery, hospital accommodation and diagnostic tests such as MRI, CT and PET scans. You choose Comprehensive Cancer Cover or the lower-cost NHS Cancer Support option.
Diagnostics Only
A cheaper alternative that pays for specialist consultations and diagnostic tests, then leaves treatment to the NHS. Useful for tight budgets.
Out-patient options
Standard includes up to two specialist consultations a year, Enhanced up to four, and Full is unlimited, with diagnostics covered in each.
Therapies
Physiotherapy, osteopathy, chiropractic treatment and acupuncture, usually up to a maximum of 10 sessions per year.
Further options include mental health cover, a private GP option, travel cover and an employee assistance programme. Always check the current policy wording, because AXA updates its benefits and yearly limit figures from time to time.
How AXA supports your employees day to day
Much of the value of the plan comes before anyone makes an insurance claim. Most plans include:
- Doctor at Hand. A 24/7 online GP service by phone or video, so employees can book a GP appointment without waiting a week.
- Health Support Line. A 24/7 line staffed by nurses and counsellors, which can also include mental health support.
- Muscle, bone and joint support. Employees can access care from a physiotherapist without a GP referral, which speeds up physio for back and joint problems.
- Specialist booking. A service that finds and books specialist consultations for your employees.
- Cancer and heart support nurses for employees going through serious treatment.

AXA's hospital list and the Guided Option
Unlike some insurers, AXA gives all business clients access to its full hospital directory across the UK, so you do not usually need to choose a restricted hospital list to control cost. That suits employers with staff spread around the country.
The Guided Option is AXA's main way to reduce the premium. When an employee needs to see a specialist, AXA offers a shortlist of up to three specialists and arranges the appointment. You save money in exchange for a little less choice. It is not available with every option, so we check it against the rest of your plan.
Standard cover
Employees choose from the full hospital directory, with a lower out-patient limit and a higher excess to control cost.
Comprehensive cover
Full out-patient cover, Comprehensive Cancer Cover, therapies and mental health, for employers who want the strongest benefit.
Does AXA cover pre-existing conditions?
It depends on the underwriting you choose. AXA offers four ways to underwrite a business policy:
- Moratorium. No health questions at the start. Conditions from the five years before joining are excluded until the employee has had two clear years on the plan.
- Full medical underwriting. Each employee declares their medical history, and any exclusion is set out in writing from day one.
- Continued medical exclusions. For businesses switching from another insurer, carrying over existing terms so nobody loses cover.
- Medical history disregarded. For groups of 15 or more, pre-existing conditions are automatically covered, subject to the policy terms.
Chronic conditions, routine monitoring, cosmetic treatment and emergency care in A&E are generally not covered by any company plan, AXA's included. Private health insurance is for the diagnosis and treatment of medical conditions that are acute and new.
How much does AXA business health insurance cost in 2026?
AXA prices each business separately. The biggest factors in the premium are:
- the number of employees and their ages;
- where your team lives, with central London the most expensive;
- whether you choose Treatment or Diagnostics Only;
- your out-patient level, cancer option and therapies;
- the excess and the underwriting you choose;
- whether you pay monthly or annually, since paying yearly is usually cheaper.
Illustrative only, small businesses in Northamptonshire and Bedfordshire commonly pay somewhere between around £35 and £140 per employee per month, depending on age and options. The only reliable figure is a quote.
How to keep the premium down
Choosing a £200 excess per policy year, the Guided Option or NHS Cancer Support can all reduce the premium. So can Diagnostics Only for younger teams. The right level of cover is a balance between what your employees would actually claim for and the risk of a claim you are happy to leave uninsured.
Can a limited company pay for it?
Yes. Premiums are normally an allowable business expense for a limited company. Employees pay tax on the benefit through their P11D, and the employer pays Class 1A National Insurance.
Does AXA Business cover mental health?
Mental health cover is an option you can add. It pays for in-patient, day-patient and out-patient mental health treatment, including sessions of cognitive behavioural therapy, up to the limits of the plan. Without it, employees still have the Health Support Line and any employee assistance programme you add, but not the treatment itself.
Given how much time off work now comes from stress, anxiety and depression, most of the employers we work with include it. Compare the limits carefully, because this is where insurers differ most.

An NHS GP or AXA's Doctor at Hand service gives an open referral.
2. Call or go online
The employee contacts AXA to check cover before treatment. This is the moment to confirm the claim.
3. Book the specialist
AXA or the employee books the consultation and diagnostic tests.
4. Pay the excess
The employee pays any excess once per year. AXA settles the rest with the hospital.

Is AXA the best company health insurance for a small business?
For many small firms, AXA is one of the strongest options. Its full hospital directory, modular design and musculoskeletal support make it easy to tailor your policy to the cover you need. It is a particular fit for employers who want to customise cover tier by tier rather than pick from a few fixed packages.
It is not automatically the cheapest, and every team is different. Bupa may suit employers who value its brand and its own clinics; Vitality suits engaged teams who will use its rewards; Aviva is often keenly priced. AXA group health insurance as your team grows
As a business grows, AXA's group health options change with it. Smaller teams are priced on the average age and location of the staff cover, while larger groups can move to corporate schemes priced partly on their own claims. That can reduce the cost of your business health benefit if your team is healthy and claims are low, but it can also push premiums up after a bad year.
Many employers use AXA group health as one part of wider employee benefits, alongside group income protection and death in service. Together they give a strong health and wellbeing package and meaningful cover for employees and their families. If you want cover for employees only, directors only or everyone, AXA can accommodate it, and you can choose whether to cover your employees' partners and children too.
One point of confusion worth clearing up: AXA Health, which provides this cover, is separate from AXA UK's general business insurance range. A business insurance policy from AXA UK, such as public liability or legal expenses cover, has nothing to do with health cover for your staff.
